The UnitedHealth Group, one of the largest commercial insurers, told analysts that so far this year, insured hospital stays actually decreased in some instances. In reporting its earnings last week, Cigna, another insurer, talked about the ?low level of medical use.?

As everyone from individual policy holders to corporate benefit programs turn to high deductible plans in the effort to lower the high cost of insurance premiums, policy holders find themselves responsible to pay medical bills of anywhere from $1,000 to $10,000 before their insurance kicks in.

When that kind of money is coming out of one?s pocket, people tend to think long and hard before heading off to the doctor.

?I am noticing my patients with insurance are more interested in costs,? said Dr. Jim King, a family practice physician in rural Tennessee. ?Gas prices are going up, food prices are going up. They are deciding to put some of their health care off.? A patient might decide not to drive the 50 miles necessary to see a specialist because of the cost of gas, he said.

But Dr. King said patients were also being more thoughtful about their needs. Fewer are asking for an MRI as soon as they have a bad headache. ?People are realizing that this is my money, even if I?m not writing a check,? he said.?

I can hear the distant cheers of the consumer driven health care movement, peppered with the more than occasional ?I told you so.?

And yet, the Regina Hertzlinger brigade might wish to hang back a bit as the other shoe has yet to drop.

It will drop ? and when it does, you are not going to like it.

The ?free-markets in medicine? forces have long argued that when people are responsible to pay for a meaningful chunk of their own medical bills, they will be more selective about how they choose to spend their hard earned cash.

The recent data would certainly appear to bear this out.

However, the argument distracts from the real truth behind high deductible insurance programs, a truth which reveals that these plans are hugely punitive for all but the wealthiest in our country.

While the rich tend to purchase insurance policies with lower deductibles, because they can better afford the premiums, lower income groups are forced to buy the higher deductible plans if they are to have any coverage at all.?The result is that those with high deductible plans shy away from spending enough on health care to cover their deductibles? ? unless they are confronted with a major illness ? as the out-of-pocket expenditures become difficult, if not impossible, for them to handle.

That means that the insurance companies get to take these lower premiums and pocket them as profit because there is less likelihood that the policy owners are going to reach the deductible limit requiring a pay-out by their insurance company.

This works out great for the publicly traded insurance companies where management tends to focus on the large bonuses that come with large increases in annual profits.

But here is the part of the puzzle that free-marketers never wish to face up to ? almost every serious medical condition begins as a less serious medical condition.

If that less serious illness is discovered and treated early, both lives and larges sums of money are saved ? not only by the insurance companies but also by typical policy holders that will still find themselves paying 20% or more of some hellacious medical charges even when their insurance does kick in.

Thus, as the public avoids the doctor today in an effort to hang onto their money to pay for more immediate expenses of life, for many, this will prove to be a short-term solution that will end up in medical and financial disaster.

Don?t believe it? You should. The insurance companies certainly do.

Despite the record profits, health insurers continue to raise premiums.

While they claim that the increases are in preparation for the day when the economy improves and consumers return to caring for their health, this is an unlikely explanation.

What the insurers realize is that this lack of attention to present medical needs, preventative and otherwise, will result in higher-cost treatments tomorrow ? treatments that the health insurers will have to pay for.? But in a world where today?s profits are what matters, the pending health crisis many will face is a topic for another day, unless you happen to be the one who experiences the health crisis that could have been averted.

When this other shoe drops ? and the insurers know that it will ? the insurance companies will conveniently forget that they raised their prices in preparation for this eventuality and will, again, raise their premium charges to deal with the swelling use of medical services that must result from avoiding needed care early on.

What we see is that the insurance companies will profit, one way or another, until the public figures out that the catastrophic policies they are buying are little more than false security being peddled in the name of holding down insurance policy premiums.

Does this sound like an acceptable long-term payer policy for anyone but the wealthiest among us?

It?s not ? unless you happen to be a health insurance company.

You think you've heard enough about the cloud? I get pummeled daily by phone calls and email pitching every possible cloud angle. If there were a cloud toaster, the UPS guy would bring me one.

But even I was surprised to see three new cloud studies released in the space of a couple of weeks -- from Harvard Business Review (HBR), The Open Group, and Quest Software.

In the midst of cloud overkill, I suddenly had an opportunity to mash together a big survey base of about 2,500 respondents and identify some significant trends. First, let's look at each survey's signature result:

  • In the HBR cloud survey, which targeted "business and technology leaders," the surprise was that flexibility, scalability, and the ability to experiment were the big attractors -- not cost reduction, as you might expect.
  • The Open Group survey, which targeted "IT professionals," highlighted a disconnect: 82 percent of those surveyed said that they expect the cloud to impact "one or more business processes," but only 28 percent were prepared for the changes that might ensue.
  • The Quest survey, which targeted "IT professionals in government and education," found that half of respondents were already on the cloud adoption path -- but roughly that same proportion said they had no "exit strategy" if their cloud initiative were to fail.

All this serves to remind us that when it comes to the cloud, we're still in very early days. I think that's one reason the surveys themselves are a little sketchy. Are answers to broad questions about "cloud adoption" useful, when that can mean anything from a Salesforce.com account to a private dev and test cloud?

I would argue that they are -- but only in measuring attitudes, not implementations of anything. In one stark assessment of the muddled nomenclature, nearly two-thirds of respondents to a question in the Quest survey saw "a lot" or a "moderate level of" confusion about the difference between cloud computing and virtualization.

Afghans have plenty of serious issues to contend with, but a surprising number of them are concerned with ... a number: 39.

Many cultures consider certain numbers to signify luck or disaster. In Afghanistan, 39 is taboo.

It's hard to find a credible story to explain what exactly it means, but everyone knows it's bad. Many Afghans say that the number 39 translates into morda-gow, which literally means "dead cow" but is also a well-known slang term for a procurer of prostitutes ? a pimp.

In Afghanistan, being called a pimp is offensive, and calling someone a pimp could carry deadly consequences. Similarly, being associated with the number 39 ? whether it's on a vehicle license plate, an apartment number or a post office box ? is considered a great shame. And some people will go to great lengths to avoid it.

"I recently bought a new Toyota car. Every time I drive to my new home in Kabul, kids call me '39,' " says Arif Zahir, 30-year-old Kabul resident. "It is because of my license plate, which has the number 39."

Zahir says that his car cost him $16,000, but the 39 plate has reduced its value by half.

Origins In Herat?

Some people say a famous ne'er-do-well in the western Afghan province of Herat had 39 on his car, and now anyone with that number faces everything from snickering to outright disdain.

Others say it's been a taboo for years, and the arrival of mobile phones and increasing access to the Internet has spread it to other cities in Afghanistan.

"In Herat, the number 39 has been a taboo for years. People have changed their phone numbers, addresses and now it has hit car owners," says resident Mohammad Tamim Abdullah. "In Herat, when the vehicle registration number reached 39,000, people kept waiting for months for it to pass this number before they would apply for new license plates."

An Issue For The Authorities

Traffic authorities in Kabul blame car dealerships for starting the craze.

"Car dealerships and those who work for the mafia started the rumors about 39 so they could buy cars with 39 plates cheaper and sell them back for higher prices after changing the plates," says Abdul Qader Samoonwal, an official at Kabul's Traffic and License Registration department.

Abdul Matin, who owns a car dealership in Kabul, rejects such allegations. "We don't know how the taboo about the number 39 started, but it's not the car dealerships," he says.

The head of Kabul's motor and traffic department recently organized roundtable discussions to dispel any taboo with the number 39. He went on Afghanistan's TV channels with prominent imams and a controversial but well-known Afghan mathematician, Sediq Afghan.

"We warned people that any word, offensive or otherwise, could come up if we use numerology," Afghan said.

The imams also warned that the Quran, the Muslim holy book, contains verses that are numbered at 39. Associating the number with pimps "is a sin because 57 Suras from our Quran contain the number 39," Afghan said.

Yet it does not seem to have allayed the fears of many Afghans. In fact, some Kabul residents say that a warlord running for Parliament in last year's election attacked two people after he was called 39. Some say he had 39 in his ballot number.

Coping With 39

Some drivers have come up with creative ways to hide 39 in their license plates. They punch holes in the middle of the number "3" to make it look like "8." Others cover their license plates with a blue or gray sheet of plastic. Some have even painted over the number in white.

Waseh, an Afghan driver with an international nongovernmental organization in Kabul, says he was recently shocked to discover that one of the cars assigned to him has a license plate number that begins with 39.

"I have no choice but to drive this car since I earn my living working here," Waseh says. "But I have to cover the 39 plate with a blue sheet. I do this to protect the dignity of this organization and also of myself."

Waseh is also facing another problem: He is 39 years old. When asked how he copes, he says he tells people that he is yak kam chehl ? one year till 40.

As Afghanistan welcomed the New Year and a new decade on March 21, some were not sure what to make of the year's numerical composition: It's 1390 in Afghanistan, and that means Afghans have to either cope with it or forget the fact that they will be stuck with a 39 in their calendar for a whole decade.

A couple a weeks ago, I attended the UK?s leading event on everything web Internet World (#iwexpo) which took place over three days at Earls Court; London. ?Right now every industry from e-learning to aerospace seems set on a collision course with social software. ?Many companies are asking themselves if they have what it takes to compete in the social media marketplace. Who, after all, has the time or the budget to mimic Disney or Starbucks? Certainly not a small or medium sized business!

Like it or not customers are talking about you online, are you there to listen and respond? If not, this is a wasted opportunity and one that could cost you in the future. Twitter hasn?t replaced the telephone for customer support yet, but the list of companies using it as a customer support channel is growing.

It struck me listening to the conversations at the show that customer service is a good place to start in social media. Customers and your employees have more power than ever before. Mobile, social, video, and cloud technologies give individuals tremendous access to information and resources. Forrester talks about this as an ?empowering?? the customer and companies are scrambling to find ways to keep these customers satisfied.

To succeed in this era of empowered customers, each employee should be encouraged to think about and solve customer and business problems. No company is nimble enough to serve or deliver 100% customer satisfaction using traditional channels. With established processes and departmental boundaries, things move too slowly. Every employee can help to solve customer problems.

Taking a view that customer service is a form of marketing means a change in approach for both disciplines and requires effective collaboration between both departments. Don?t throw away customer service tools and systems, or tear up marketing plans. The key to turning customer service into a marketing channel is to be efficient in assisting employees to solve customer problems, drawing in expertise from every department. This is much harder than it sounds. It means driven staff needs to be coming up with solutions using their own initiative. Management?s new job is to support motivated and talented employees. IT?s new job is to support and encourage employees.

This may sound like a complete inversion of the top-down way companies operate, but customer service that features outreach through social channels can energise word of mouth from those happier customers. Companies need to learn how to leverage social media to build their brand images. It?s important not to over interpret ?influence? from individuals, put this aside, all customers? count (not to mention the fact that?influencers are overrated). A company?s job is to keep its all customers happy. Responding to complaints on Twitter and other social media can be a great way to do that.

Why not kick start social media efforts in an area that will show some real ROI like customer service? ?Your first action is to create a set of social media policies and guidelines,?provide your business a framework to carry out your social media strategy?and tactics. ?Then you will need to report on your progress. A tip I picked up from Jeremiah Owyang of Altimeter research group is to avoid information overload of executives and management, Owyang offers?up a valuable solution, provide the right metrics to the right audience. Know who your internal audience is and give them the appropriate information.
